What breaks: the lookup nobody owned

A code added at the client and never added to the lookup, and a lookup edited in production with no record of who or why. THE FIRST is the ordinary failure and it is not really a technical one. Finance adds a new payment term, or a new supplier category, or a new tax code. The lookup does not have it. The next transaction carrying that code arrives at a mapping that finds no match — and what happens next depends entirely on how the lookup was written: it fails, or it silently passes an empty value through to the target, which is worse. The lesson is that a lookup creates a DEPENDENCY ON A HUMAN PROCESS, and nobody was told. Somebody at the client can now break an integration by doing their ordinary job correctly.
