Six cases, and the first is both the most common and the most serious, which is an unusual combination. One. "Everyone in HR can see everything." One generic document type, no type security profiles — the default from the first lesson, arrived at by omission rather than by decision. Two. "A manager saw a document they shouldn't have." Manager access on the type was never questioned; not decided wrongly, never asked. Three. "We can't change the document type now." An attribute locked after records exist. Four. "We purged records we needed." Purge is irreversible and was run against the wrong rule — there is no recovery paragraph here, because there is no recovery. Five. "Visas expired and nobody knew." Expiry dates configured, no report built. Six. "Retention was never set and we have eleven years of records." A design omission, expensive and risky to remediate, and the risk accumulates the
