Eight cases. One. "The second load created duplicates instead of updating." Key strategy wrong — understand source keys before the first load, because by the time you discover this the second load has already happened. Two. "Assignments failed because departments did not exist." Sequence; use the runbook. Three. "Everything failed on effective dates." The most common first-load failure by a wide margin — expect it, and check the dates before you check anything else. Four. "Managers are missing." The hierarchy pass was not run, or was run before all the workers existed. Five. "It loaded fine in test and failed in production." Volume, or configuration differences; prevention is a full-volume dry run, not a sample in test and a hope for production. Six. "Headcount is off by nine."
