The tree version was not activated, or not published — two different misses with the same symptom, checked in that order. Labour costs are reporting to the wrong cost centre. The department-to-cost-centre mapping is wrong — and Finance notices before HR does, which means you hear about it from somebody who is already annoyed and does not consider it an HR system problem. Public holidays applied to the wrong workers. Location setup or coverage, surfacing much later, by which point you will have forgotten that a location decision caused it. Approvals go nowhere.
