Six cases. One. "Backflush consumed from the wrong subinventory." The work center supply subinventory — and the symptom is an inventory discrepancy, not an error. Two. "Capacity is half what it should be." The shift pattern — or utilization and efficiency stacking unexpectedly. Two candidates, and the second is the one people do not think of, because each percentage looked reasonable on its own. Three. "Scheduling ignores a machine being down." No maintenance exception on the resource. Four. "The controller wants cost by machine and we cannot produce it." Granularity decided without them — not fixable without restructuring and re-costing, and it arrives in month nine as a request that cannot be met. Five. "Fifty work centers and operators cannot find their work." Over-modelled — a usability failure, and note that nothing is technically wrong: the model is accurate, and simply unusable by the people who have to use it…
