Seven failures, and they divide cleanly into things you prevent with a process step and things you prevent with a design conversation. The manager was terminated and their team has no manager. Orphaned reports. Prevention: a reassignment step in the termination process — one line in a procedure, and it removes the whole failure. An approval has been stuck for three weeks. No escalation or delegation configured: somebody is on leave, or has left, and the transaction is sitting in their queue being invisible. An approval notification exposed compensation. Secured notifications not configured. This is the serious one on the list — security you configured in an earlier module, defeated by an email. The service date is wrong after a country transfer.
